Medically Tailored Meals
Medically Tailored Meals (MTM) is a therapeutic nutrition intervention aimed at improving health outcomes and reducing hospital readmission rates. Meals are prepared for and delivered to clients living with an acute or chronic illness. This service is offered through a referral from a medical professional or healthcare plan as well as a self-referral process. Each meal plan is specifically tailored to meet the nutritional needs and intolerances of the patient by a Registered Dietitian.
We are currently serving clients thoughout Ventura County through County of Ventura AAA and Gold Coast Insurance /Medi-cal for Ventura County.

Fresh & Fabulous is a registered dietitian-owned and operated catering company providing fresh diet-specific meals direct to patient's doors anywhere in Ventura County. We contract with the Ventura County Area Agency on Aging to provide meals to referred Gold Coast clients.
Our team of dietitians not only hold degrees in dietetics, they also have years of experience in consulting to senior nutrition programs, restaurant ownership, hospital foodservice management, and catering senior home delivered meals and Medically Tailored Meals. Our team's experience is crucial in providing delicious, quality meals that help your patients get better faster.
Meals tailored to your patients:
Our team of Dietitians contact your patient and discuss any other dietary issues, allergies, likes and dislikes to tailor a menu that meets their nutritional needs and is also well accepted.
Providers may refer eligible members by using the Gold Coast MTM Authorization Request Form
